**Job Title: 3rd shift Full-time Nurse/QMA**

**Job Summary:**
Chandler Place is seeking a passionate and dedicated Licensed Practical Nurse (LPN), or Qualified Medication Aide (QMA) to join our dynamic healthcare team where you feel more like family! The ideal candidate will possess a strong commitment to high-quality patient care and demonstrate the ability to work effectively in a fast-paced environment. This position requires exemplary attendance and dedication to going above and beyond every day for our resident's, families, and co-workers.
**Key Responsibilities:**
- Provide compassionate and personalized nursing care to residents in accordance with established protocols and standards.
- Administer medications and perform routine procedures following physicians' orders and best nursing practices.
- Monitor resident health, recording and communicating any changes to the healthcare team to ensure optimal patient care.
- Collaborate with physicians, healthcare professionals, and residents/families to develop and coordinate effective care plans in a home like environment
- Assist in maintaining a safe, clean, and therapeutic environment with a hometown feel for residents and staff.
- Maintain accurate and detailed patient records, ensuring confidentiality and compliance with all applicable regulations.
- Educate residents and their families on health management and disease prevention strategies.
**Qualifications:**
- Valid state licensure as an LPN, or certification as a QMA.
- Previous experience in a Assisted Living or Skilled Nursing setting is preferred.
- Strong interpersonal and communication skills.
- Ability to work independently and as part of a team.
- Excellent organizational and multitasking abilities.
- Commitment to work scheduled shifts as determined by the community needs.
